Photo and Recording Consent Form

By typing “YES”  on the Registration Form (here), I, [Parent/Guardian Full Name], authorize the Summer Abacus Club and its representatives to take photographs and audio/video recordings of my child, [who I have listed on the Registration Form Here], during club sessions and activities. I grant permission for these images and recordings to be used for educational, promotional, and informational purposes related to the activities of the Summer Abacus Club. This may include, but is not limited to, use in print, on the internet, and all other forms of media.

I understand that my child’s participation in photographs and recordings is voluntary and that I can withdraw my consent at any time by notifying the club organizers in writing. I also acknowledge that participation in club activities is not contingent upon this photo and recording consent.
